ABOUT AIOS AIOS is building the world’s first full-stack AI doctor. We’re at $400M ARR, growing from $10M/yr 12 months ago. We’re the world’s fastest growing AI doctor. We’re faithfully serving >200k/mo patients via Bolt Pharmacy, our main UK brand. We’re profitable. Our strategy exists at the intersection of two strong theses: 1. The AI doctor that wins will get to escape velocity using GLP-1s, the fastest growing consumer product in history. 2. The $2T European healthcare market is overlooked by the most talented builders. Our master plan: - Step 0 → $100M/yr by end of 2025: We went from $10M to $100M in 6 months serving the UK GLP-1 market. - Step 1 → $1B ARR by end of 2026: Over the last 12 months we've grown from 3k to 150k UK active GLP-1 patients. We’ll continue this growth curve to hit $1B ARR. - Step 2 → $10B ARR by end of 2028: Blitzscale Europe. We’ll be Europe’s largest GLP-1 provider. - Step 3 → $100B/yr by end of 2031: Get regulatory approval across Europe for our Full Autonomous Prescribing (FAP) system. Win contracts at scale with European payers to mass replace human labor. We’ll be the dominant full-stack AI doctor in Europe. - Step 4 → $1T/yr by end of 2035: With one line of code and zero human time, you can use AIOS to treat any patient globally with any medication. In so doing, we’ll become the world’s first trillion-dollar healthcare company. We’re a young, founder-led company. This is still Day 1 and all our work is ahead of us. Our systems serve >200k patients each month. Our agentic workflows autonomously generate >$100k of revenue per day. Our customer products support complete healthcare journeys, while our clinical systems help clinicians deliver care safely at enormous scale. You’ll join one of these teams: - AI Growth: Build self-improving growth loops across visitors, leads, and prospective customers. You’ll work across customer-facing AI, internal automation, experimentation, and growth products. - Product Growth & Retention: Make checking in, reordering, and continuing treatment effortless. You’ll build in React Native and help make our mobile app the core product customers rely on throughout treatment. - Engagement: Build the best healthcare app in the UK for patients and non-patients alike. You’ll create useful, engaging products that give people a reason to open the app regularly and build a lasting relationship with our brand. - Care & Aegis: Build the clinical workflows, tools, data flows, and backend systems surrounding Aegis, our AI-assisted clinical decision-support system. During the interview process, we’ll understand where you do your best work and find the team where your strengths can create the most leverage. You’ll share ownership of the product itself: what gets built, how it works, how it feels to use, and whether it produces the outcome we expected. You’ll understand the people using your work, collaborate closely with product and design, and question decisions that do not make sense. Your ownership continues in production. You’ll follow how people use what you built, understand the relevant metrics, identify where the experience falls short, and keep improving it. We care deeply about the quality of our software. The systems you build must be clear, tested, observable, and maintainable by the engineers who come after you. You’ll make thoughtful architectural decisions, handle failure cases properly, review code seriously, and strengthen weak foundations before they become permanent constraints. Speed matters just as much. At our pace, excellent engineering means finding the simplest sound design and shipping it quickly. Move fast, build well, and leave foundations that can carry what comes next. We provide Claude Code and Codex subscriptions. We expect you to use them to increase your speed and ambition while retaining complete responsibility for your judgment, understanding, and work. This role is FTE and remote, with some in-person working days with the Kosovo engineering team. You’ll report directly to me, Gzim https://www.linkedin.com/in/ghels/ :) (VP of Engineering). You’ll also work closely with: - Ben Dowdle https://www.linkedin.com/in/bendowdle/ (Head of Product) - Richie Cartwright https://www.linkedin.com/in/richard-cartwright/ (CEO) KEY RESPONSIBILITIES - Build Products End to End: You’ll work closely with product and design to understand the problem, shape the solution, build it across the stack, and carry it through production. - Own the Experience: You’ll care about the complete experience, including edge cases, loading states, performance, reliability, and the small details that determine whether a product feels excellent. - Build to Last: You’ll write clear, tested, observable, and maintainable software. You’ll choose simple architectures, handle failure cases properly, and strengthen the foundations beneath your work. - Own Production: You’ll instrument what you ship, follow the relevant metrics, investigate failures, and keep iterating until the product creates the intended outcome. - Use AI as a Force Multiplier: You’ll use Claude Code, Codex, and other AI tools throughout development to move faster, explore more ambitious solutions, and increase the quality of your work. - Raise the Engineering Bar: You’ll review code seriously, communicate decisions clearly, share context, improve weak systems, and help the engineers around you do better work. NEED TO HAVE - Experience: You have 3+ years of professional software engineering experience and have shipped meaningful products used by real customers. - Full-Stack Engineering: You can deliver across frontend and backend systems. You may be significantly stronger on one side, but you are comfortable working across both. - Product Mindset: You care about what should be built, how it feels to use, and whether it creates the intended outcome. - Engineering Quality: You write clear, tested, observable, and maintainable software. You understand architecture, databases, APIs, failure modes, and production reliability. - Ownership: You acquire context proactively, raise uncertainty early, follow your work into production, and take responsibility for reaching the outcome. - Judgment: You can balance speed, simplicity, and long-term maintainability. You push back clearly when a technical or product decision does not make sense. - AI-Native Execution: You use AI development tools effectively and remain fully accountable for the quality and correctness of the work they help you produce. NICE TO HAVE - TypeScript: You have strong experience building production systems with TypeScript. - Our Stack: You’ve worked with technologies such as React, React Native, Expo, Next.js, NestJS, GraphQL, Prisma, PostgreSQL, Redis, queues, and AWS. - Note: Strong React Native experience is required for the Product Growth & Retention and Engagement openings. - Applied AI: You’ve built customer-facing products, internal tools, or automated workflows on top of LLMs. - Experimentation: You’ve instrumented products, run experiments, and used production data to decide what to improve next. - Product Taste: You care about UX details and work effectively with designers to produce thoughtful, polished experiences. - Startup Experience: You’ve worked in an environment where speed, ambiguity, and individual ownership were real. - Education: A bachelor’s degree in Computer Science or a closely related technical field is a strong advantage. OUR CULTURAL STANDARDS We aim to make this your life's work. This should be the most challenging, most rewarding role of your life. Accordingly, these are the core cultural standards to which we hold ourselves & our team-members: 1. Belief in the mission: We will have served 100 million patients by the end of 2035 and we transform the life of most patients who join. We have a lot of work to do. We are obsessed with our patients and are dedicated to the mission. 2. Unwavering integrity: We are at the frontier, so we often live in ambiguity with no trodden path. When we can't look to others for guidance, we must maintain impeccable ethics and unwavering integrity. 3. Only the paranoid survive: Bad sh*t is coming. By joining us, you're choosing to sail straight towards the storms with unhesitating conviction. However much we've already done, however far we've already come — it's still Day 1 and all our work is ahead of us. 4. If we're average we fail: We are only interested in "insanely great", a focus on the quality of our execution that in everyday life would be considered pathological. We have a dedication to excellence and reject incompetence. 5. Commitment to candor: That which can be destroyed by the truth should be. You get full transparency from the company and the company expects full transparency from you. We never say anything about someone that we wouldn't say to them directly. We give feedback with love and do not need to protect people from fleeting physical sensations. 6. A maniacal sense of urgency: We execute at an intensity that most people think is impossible. Speed is critical and we need things done yesterday. We all work very hard and in such a competitive world there really is no other way to win. 7. Enduring frugality: We are frugal. We hate being wasteful and we are anti-luxury. A culture of cheapness keeps us young. We spend our cash wisely & carefully — in a way that would make our grandmas proud. 8. Bulldozing barriers: The world is malleable and we shape it. We truly believe this and act accordingly. We are relentlessly resourceful and are at the mercy of no-one but ourselves. You'll be shocked how capable you are and how much you can achieve. 9. Keep your head down: We're boring people doing exciting work. We don't chase short-term status — we ignore short-term dopamine hits and focus on what matters. Outsiders will underestimate us and we revel in that. 10. The power of focus: We live in a world of power laws and we cannot overestimate the unimportance of practically everything. Know your One Thing, and nail it. 🎯 "You just build a f\*ing amazing experience. Make each step amazing. Make every decision in the long term interest of the customer. Give the customer massively more value than you take." COMPENSATION & BENEFITS You get paid above market salary and you get early stage equity, so you get really rich if we nail this. - Compensation: : €40k-€80k, + equity BENEFITS The stuff below is cool as well. - Healthcare: comprehensive medical insurance - Vacation: PTO with a yearly minimum (≥2wks/yr + local national holidays) - Remote: our team is fully distributed across the world and functions fully remotely - Personal development: budget for books, courses, coaching ($1200/yr) - Personal wellness: budget for gym, health apps ($1200/yr) - Coaching: free biweekly health coaching - Equipment: Macbook & work-from-home equipment provided as needed - What are we missing? We're still early so you get to shape our culture.
